Required Health Exams & Immunizations

Physicals and proof of immunizations are due upon entry for Preschool and Early Childhood Education Programs, Kindergarten and 6th grade. A physical and proof of immunizations are also required for new/transfer students to the District. Illinois law requires that each student submit the following requirements upon starting school for the first time in District 26. The School Code states that children will be excluded from school if the requirements for physical examinations and immunizations are not met.

Los exámenes físicos y el comprobante de vacunas se deben presentar al ingresar a los Programas de Educación Preescolar y Preescolar, Kindergarten y 6º grado. Un examen físico y prueba de inmunizaciones también son requeridos para estudiantes nuevos/transferidos al Distrito. La ley de Illinois requiere que cada estudiante presente los siguientes requisitos al comenzar la escuela por primera vez en el Distrito 26. El Código Escolar establece que los niños serán excluidos de la escuela si no cumplen con los requisitos de exámenes físicos e inmunizaciones.

1. State of Illinois Physical Examination Form entitled, “Certificate of Child Health Examination", dated within one year after the first day of the previous school year. Parents complete and sign Health History Section.
2. Complete Immunization record as outlined by the Illinois Department of Public Health.
Prior to October 15- DTP – 4 doses
- Polio –3 doses
- Hepatitis B-3 doses at appropriate interval
- MMR – 1 dose
- Varicella – 1 dose ○ Hib (Haemophilus Influenzae Type B) – varied schedule – 3-4 doses
- Pneumococcal – varied schedule 3-4 doses

Required Exclusion (Section 27-8.1 of the School Code)
Students who do not submit Proof of Immunizations and Physical Examination by October 15 are required by law to be excluded from school.
Lead Screening Law (P.A.-93-0104)
For students 6 months through 6 years of age, the health care provider must conduct a lead assessment survey to determine if a lead screening is necessary. Children over 6 years of age may be screened in conjunction with the school health examination when the health care provider determines that the child is potentially at high risk for lead poisoning.
