Please see school health exam requirements in the table below, specified by grade.
State law for all Illinois school children mandates submission of these forms. Please schedule appointments early and utilize the required forms. Additional health examinations may be required when deemed necessary by school authorities. If you have questions about these requirements, please contact the school nurse at your student’s school building. More specific information about the required immunizations is on this Illinois Department of Public Health web page.
Required Exclusion (Section 27-8.1 of the School Code): Students in grades requiring submission of the Certificate of Child Health Examination Form are required by law to be excluded from school if the Form, or a Religious or Medical Exemption, is not received by October 15th of the current school year.

1. State of Illinois Physical Examination Form entitled, “Certificate of Child Health Examination.” This form is compliant for 12 months after completion.
2. IHSA Form entitled, “Pre-participation Examination”. This form is compliant for 13 months after completion.

Options to submit forms:
1. Forms may be faxed to a secure fax at (847) 516-9705.
2. Bring forms in person to your building nurse.
3. Mail forms via USPS to “Building Nurse” at your school’s address.Medication Administration
Medication shall be administered only when absolutely necessary to maintain the continued attendance of the student. The Authorization for Administration of Medication at School form requires a doctor’s signature and a parent’s signature before medication can be brought to school.
