Legislation dealing with school health records and immunization requirements for all Illinois school children mandates that each student upon initial entrance to District 26 schools have a
Physical examination and a documented immunization record in compliance with
current public health regulation.
Dental examination (required for all children entering kindergarten)
Eye examination by an optometrist or licensed physician
(new State law effective 1/1/08)
Under the Lead Screening Law (P.A.-93-0104), your health care provider must conduct a lead assessment survey to determine if a lead screening is necessary. This requirement is for children 6 months through 6 years of age. Children over 6 years of age may be screened in conjunction with the school health examination when in the medical judgment of the health care provider, the child(ren) is potentially at high risk for lead poisoning. The attached "Certificate of Child Health Examination" (physical form), “Proof of School Dental Examination Form”, and the “State of Illinois Eye Examination Report” are the only correct forms to use. Please make early appointments for your child/children, and have your licensed health care provider complete all areas and sign each form. Parents/guardians are to complete and sign the health history section of the physical form. The School Code states that children will be excluded from school if the requirements for physical examinations and immunizations are not met.
Please return the forms to your child’s school no later than two weeks before the first day of school. Incomplete forms will be returned to parent/guardian.
